Please note: There is an updated version of the RIIL "Assumption of Risk" form that must be signed by ALL  NEW ATHLETES and handed in to the Athletic Secretary at the beginning of the 2023-2024 athletic season.

Physical Examination

Students registering for Athletics must have a current physical examination on file in the Athletic office before the student is authorized to participate in practices/tryouts. To ensure clearance for participation, a hard copy of the current physical MUST be handed in to the Athletic Secretary. Also, on the Physical Form within the registration, you must enter the date of the physical in the box requesting it. If you do not enter the information required, it will not allow you to move onto the next section. 

Physicals are valid for 1 year from the date of the last exam.

School Insurance:   Proof of Insurance MUST be indicated on registration form.  Go to the link provided below to register for School insurance.
